1. Overview of the Fee Changes
If you are planning to register for the USMLE Step 1 or Step 2 CK exams, it’s important to stay updated. Starting from January 1, 2025, the Educational Commission for Foreign Medical Graduates (ECFMG) will implement new fee structures.
- USMLE Step 1: $1,020
- International Test Delivery Surcharge: $205
- USMLE Step 2 CK: $1,020
- International Test Delivery Surcharge: $230
These changes apply to applications submitted on or after January 1, 2025.

- Inflation & Operational Costs: Rising administrative and staffing expenses.
- Enhanced Services: Continuous improvement of exam materials and support.
- Global Testing Infrastructure: High-quality international test centers.
3. Who is Affected by the Fee Increase?
- International Medical Graduates (IMGs): Applying for ECFMG Certification after January 1, 2025.
- U.S. Test-Takers: Those using ECFMG services instead of NBME.
